CONFIDENTIALITY
All clients of Maven Medical Arts are assured of complete confidentiality regarding all aspects of their treatments and services, as per our HIPPA policy. You have the right to obtain a copy at your convenience at our Draper location.
APPOINTMENT RESERVATIONS + CONFIRMATIONS
Maven Medical Arts requires that a credit card number be kept on file to reserve your scheduled service. A cancellation without 24-hour notice or a No-Show will result in a charge up to $50 (depending on the type of service) to your credit card. We do not accept Instagram DM’s or Facebook Messengers as a form of cancellation or confirmation.

Appointment confirmations are sent a week before and a day before your scheduled appointment to the cell phone and email you gave us when scheduling your service. If your email/cell has changed, it is the responsibility of the patient to notify us. Not "confirming" your appointment through your appointment reminder text/email will not be considered a cancellation. By scheduling an appointment, you are agreeing to our cancellation/no-show policy.
CANCELLATIONS + NO-SHOW POLICY
Punctuality is greatly appreciated and will enable us to dedicate the appropriate amount of time to your scheduled treatment. Arriving greater than 15 minutes late may result in the rescheduling of your appointment. We have a 24-hour cancellation policy. If you are unable to make your appointment, please call us no later than 24 hours prior to your appointment to avoid a $50 cancellation fee.
If you No-Show to your appointment, you will be charged a $50 no-show fee for your missed service.
Patients arriving more than 10 minutes late may result in a shortened appointment or a cancellation if there is not enough time to complete the treatment. If your appointment is canceled due to late arrival you will be charged the appropriate Cancellation Fee.
All patients are required to complete their medical history, HIPPA, Covid, and Treatment consents upon arriving at their appointment. When you arrive at your appointment, you will be required to provide your driver's license.
REQUIRED CONSENTS + DOCUMENTS
We accept all major credit cards and cash. We do not accept business or personal checks. You may call or text us at 1-801-806-0606 to ask about alternate methods of payment.
ACCEPTED PAYMENT METHODS
All treatments, skincare, and gift cards are non-refundable.
